This form is for non emergency incidents. If your incident is an emergency and/or is happening right now DO NOT USE THIS FORM, call 911 from your telephone immediately.

The prurpose of this form is for us to collect as much information about unusual people or incidents in your community. We can only work with you to correct quality of life issues that we are aware of.

Please complete this form as accurately and completely as possible. Whatever information you have may help the Police Department with their potential investigation.
